This is our “Emergency Medical Information Form”. Please fill out this form and print two copies. These two copies are for YOUR USE ONLY. Keep one copy in your top/left jacket or vest pocket. Keep the second copy in your left saddlebag. These will only be used in an emergency situation, should it arise. The forms can be retrieved and provided to responding medical units to assist in your care.
DO NOT send these forms to OC IMRG. They are to remain under your control. If you have additional information you would like to include on your form, such as additional Emergency Contacts or Medical Conditions, please feel free to use the back of the form.

One copy goes in your left jacket/vest pocket. One copy goes in your left saddlebag. Uniformity in form location among all members makes retrieval in an emergency situation much quicker.
